How to make
a referral
Patients' home health needs can be arranged
with just a phone call or fax to Kare-In-Home.
We ensure a smooth transition to the home
care environment for each patient.
Kare-In-Home has taken steps to provide
better customer service by enhancing its
communication efforts. Kare-In-Home follows
special requests, physician preferences,
guidelines and parameters upon receipt
of referrals. An experienced home health
nurse is available, upon request, to provide
in-home coordination of services for patients
who have complex needs or require complex
procedures.
Nurses generally will provide patient
evaluations within 24 hours of referral.
Kare-In-Home’s normal business
hours are Monday through Friday from 8:00
am to 4:30 pm. However, an on-call nurse
is available 24 hours a day seven days
a week.
